OBJECTIVES

This initiative was requested by youth, parents, and healthcare teams alike. The goal is to equip young people with the tools they need and to help them gradually take control of their health condition, making the transition to adulthood—often a challenging period—easier.

It aims to empower youth, increase their involvement in their healthcare journey, and support their mental health as they move into high school and begin gaining greater independence. It also encourages open communication between youth and their parents.

The tool developed through this project will be shared by the Foundation so that other young people can benefit from it in their own settings, contributing to a sense of accomplishment among participants.
